1. Medical Authorization:
I certify that my child is in good health and able to participate in physical activities at American Made Fitness. I understand it is my responsibility to inform coaches of any health conditions, allergies, or injuries.

2. Liability Waiver:
I agree to release and hold harmless American Made Fitness, its owners, employees, and coaches from any liability for injuries or illnesses that may occur while participating in the Kids Camp activities. I understand that fitness activities carry a risk and assume full responsibility.

3. Emergency Medical Treatment:
In the event of an emergency, I authorize American Made Fitness staff to seek necessary medical treatment for my child.

4. Photo/Video Release:
I grant permission for my child to be photographed or recorded during Kids Camp activities. I allow these images to be used for promotional purposes, including social media, website, and print materials. No names or personal info will be shared.

5. Behavior & Safety:
I understand that my child must follow all safety instructions and behave respectfully toward coaches and other participants. Disruptive or unsafe behavior may result in removal from the program.
